LifeHub is not subject to the rules and regulations regarding the privacy and confidentiality of individually identifiable health information as those of the Health Insurance Portability and Accountability Act of 1996 (HIPAA) or other similar laws or regulations. However, protecting your privacy and your information is of utmost importance to us, and therefore LifeHub will follow and comply with the applicable HIPAA laws and regulations as well.
Personally Identifiable Information (PII or Personal Information) is information that can be used to identify, contact or locate you. PII does not include information that is collected anonymously (i.e., without identification of the individual user) or demographic information not connected to an identified individual. When you visit our Web site, LifeHub collects only the Personal Information that you choose to provide. LifeHub does not require any Personal Information to use the site. We collect Personal Information only if you choose to provide it during registration. At any point, you can change or delete your Personal Information by accessing your Account Profile in the LifeHub site.
We also collect non-personal information from you during registration. We do not use permanent cookies or web beacons or store any personal information in cookies on your computer. LifeHub does not use web beacons to collect any Personal Information from or about you or track your activities across Web sites generally.
AlignMark, Inc. is the sole owner of the information collected on LifeHub.com. AlignMark, Inc. collects information from our users at several different points on our Web site. AlignMark, Inc. will not sell, share, lease or rent or give your personal information to anyone.
Please note LifeHub contains links to other sites. AlignMark, Inc. and LifeHub are not responsible for the privacy policies or the content of such other Web sites. In order to use the LifeHub site, we ask you to register as an individual user through your company. Your company will provide you with a key code. Upon initial use of the site, you will be required to register as a new user and provide the key code that your company issued to you. Then you will be required to provide a user name (which is anonymous and not linked to your name), password and secret phrase to help identify your password. Your user name and password will be required each time you login to the LifeHub site. We use this information to provide relevant information from your employer to you, if applicable. In addition, we use this information only in aggregate, along with other demographic information to your employer for site statistics, such as the number of users registered and active within the site.
Upon registration, you have the choice of providing additional information (INFORMATION NOT REQUIRED) in your member profile. You can provide your name, social security number, email address and other demographic information as established by your company, such as department and title. This information is optional and is used to personalize the site for you. This information is not shared with anyone or any other entity.
For purchases made within the LifeHub site, all personal financial and other Personal Information you provide is to one of LifeHub's e-commerce partners, this transaction will occur on the e-commerce partner's site, not the LifeHub site. As such, the financial and Personally Identifiable Information you provide is collected by and controlled by the Privacy Policy of that e-commerce partner. It is important therefore that you familiarize yourself with the Privacy Policy of the e-commerce site with which you transact business.
When you post a question to our experts through "Ask The Expert" or when you request information or help from our Customer Service via the Customer Service Request Form, we ask for a valid email address. However, it is not required that you provide a valid email address to post a question or to receive a response. In such a case you may read the response at the LifeHub site. We do not use this Personal Information for any other purpose, except toreply to any questions you may have, if you choose to provide your email address.
To take part in a chat room or seminar, we ask for a nickname. However, if you are concerned about privacy, use caution when participating in chats. The chat rooms are open to all LifeHub members and therefore others using the chat rooms can read your posts. Any information that you include when participating in the chat is available to anyone with access to LifeHub. For example, if you include your email address in a chat forum, it is accessible for all LifeHub users.
Questions posted are recorded in the form of chat transcripts, which are made available on our site to all LifeHub users.
You are not required to submit any Personal Information to use the LifeHub Web site. However, you may choose to provide some Personal Information upon registration which will help personalize the service for you. In addition, the Customer Service Request Form and "Ask The Expert" provide options for email addresses to be supplied. However, email addresses are not required to use these services. For "Ask The Expert" questions, you can read a response posted to the site, if you choose not to receive an email response to your posted question. In addition, in the customer service section, you can provide a phone number or call us directly and are not required to provide Personal Information to be contacted.
All information collected by LifeHub is used to create information in the aggregate of the LifeHub users (group reports). This aggregate information is anonymous statistical demographic information used for general group reporting, but you will never be personally identified. Password is required for login. E-mail address is used to send you a registration confirmation message or to contact you when necessary. Otherwise it's only used when you specifically request information from us (such as an Ask the Expert).
We store information that we collect through session cookies and log files to create a profile of our users. A profile is stored information that we keep on individual users that detail their viewing preferences. Consequently, collected information is tied to the user via the user name which is not required to be Personal Information. If you use your surname [or provide an email] in the username, it could be identified as personally identifiable information. We collect this information to provide information on overall usage and improve the content of the site for the user. We do not share your profile with third parties.
A cookie is a piece of data stored on the user's computer tied to information about the user. Usage of a cookie is in no way linked to any personally identifiable information while on our site, unless you voluntarily use your full name as your user name or submit an email. We use session ID cookies only; once you close the browser, the cookie simply terminates.
Like most standard Web site servers we use log files. This includes internet referring/exit pages, date/time stamp, and number of clicks to analyze trends, administer the site, track user's movement in the aggregate, and gather broad demographic information for aggregate use. This information is not linked to personally identifiable information. We use a tracking utility provided in the Oracle Portal which uses log files to analyze user movement.
LifeHub realizes that children under the age of 13 may not understand all the provisions of our policy or be able to make thoughtful decisions about the choices made available. We do not knowingly collect nor require any personal information at this site for family members of an adult employee subscriber. We urge all parents to participate in their children's exploration of the Internet and any online services, and to teach their children about protecting their personal information while online.
Messages and announcements are posted to the LifeHub site weekly regarding updates to specific content areas that LifeHub publishes.
We communicate with users on a regular basis to provide requested services and in regards to issues relating to their account. We reply via email or phone, in accordance with the user's wishes.
Legal Disclaimer
Though we make every effort to preserve user privacy, we may need to disclose Personal Information when required by law wherein we have a good-faith belief that such action is necessary to comply with a current judicial proceeding, a court order or legal process served on our Web site.
Aggregate Information (non-personally identifiable)
We may share aggregated demographic information with our partners and your employer. The information is in aggregate only, such as group reports indicating the total number of active users on the site at any point in time. Other information that may be reported in the aggregate could be the number of hits per articles or domain, etc. This information is not linked to any Personally Information.
Third Party Intermediaries
We use an outside credit card processing company for purchases within the LifeHub site made with a credit card. These companies do not retain, share, store or use personally identifiable information for any secondary purposes.
Service Providers
We partner with third parties such as Amazon to provide specific services. For example, when a user chooses to buy a recommended book and links from our site to Amazon, we do not share any Personal Information with Amazon. Amazon and other partners and third parties are not allowed to use personally identifiable information except for the purpose of providing these services.
Business Transitions
In the event AlignMark, Inc. goes through a business transition, such as a merger, being acquired by another company, or selling a portion of its assets, users' Personal Information will, in most instances, is part of the assets transferred. Users will be notified via prominent notice on our Web site for 30 days prior to a change of ownership or control of their personal information. If as a result of the business transition, the users' personally identifiable information will be used in a manner different from that stated at the time of collection, users will be given a choice consistent with our notification of changes section.

We encourage our users to be aware when they leave our site and to read the privacy statements of each and every Web site that collects personally identifiable information. From time-to-time our site might request information from users via surveys. Participation in these surveys is completely voluntary. The user therefore has a choice whether or not to disclose this information. The requested information typically includes only user name and demographic information (such as department). Survey information will be used for purposes of monitoring or improving the use and satisfaction of this site. Users' personally identifiable information is not shared with third parties. Though we may use an intermediary to conduct these surveys, they may not use users' personally identifiable information for any secondary purposes.
This Web site takes every precaution to protect our users' information. When users submit sensitive information via the Web site, their information is protected both online and off-line.
When using the LifeHub Assessments, all information is encrypted and is protected with the best encryption software in the industry - SSL. While on a secure page, such as an assessment, the lock icon on the bottom of Web browsers such as Netscape Navigator and Microsoft Internet Explorer appears as locked, as opposed to un-locked, or open, when users are just 'surfing' on unencrypted pages.
While we use SSL encryption to protect sensitive information online, we also do everything in our power to protect user information off-line. All of our users' information, not just the sensitive information mentioned above, is restricted to our offices. Financial information is not collected by AlignMark or within LifeHub and therefore is not maintained by AlignMark, Inc. but rather a third party provider.
If any Personal Information is provided, only employees who need the information to perform a specific job (for example, an expert in responding to a question on an assessment) are granted access to personally identifiable information. ALL employees are kept up-to-date on our security and privacy practices. Every six months, as well as any time new policies are added, our employees are notified and/or reminded about the importance we place on privacy, and what they can do to ensure our users' information is protected. Finally, the servers that store personally identifiable information are in a secure environment.

If a user's personally identifiable information changes (name or email) or if a user no longer desires our service, we provide a way to correct, update or delete/deactivate users' personally identifiable information. If we decide to change our privacy policy, we will post those changes to this privacy statement, the homepage, and other places we deem appropriate so our users are always aware of what information we collect, how we use it, and under what circumstances, if any, we disclose it.
